This is the document library for a proposed supervised adult lactate assessment, not a testing-method article. For device operation, sampling, protocol examples and interpretation, read the Nova Lactate Plus guide. For availability and service scope, see Performance Assessment.
Review drafts only: do not sign or use for client testing. The eight original PDFs need actual provider and operating details, qualified professional review, and Pennsylvania legal/insurer review where appropriate. They are not an approved intake packet or permission to collect blood. They apply to adults aged 18 or older, not minors.
Do not send completed health forms through the public website, a booking description, public training activities or social media. Agree a secure return method first. There is no form-upload facility on this site. Keep full health histories, consent/release documents and emergency-contact records out of routine TrainingPeaks uploads; share only the reviewed performance information authorized for coaching.
Client documents
These four PDFs have different purposes. Do not substitute a waiver for informed consent or treat a health-history questionnaire as medical clearance. The health-history form already includes emergency contact details.
| Original review draft | Purpose | Before approved use |
|---|---|---|
| Adult health and training history (PDF) | Relevant history, training, sampling concerns and emergency contact; private discussion is available | Assessor reviews concerns and any screening follow-up; no automated clearance |
| Exercise and optional finger-sampling consent (PDF) | Explains procedure, risks, alternatives and stopping; exercise and sampling are separate choices | Confirm the actual protocol, competent authorized operator and safety arrangements |
| Adult risk acknowledgment and limited release (PDF) | Separate proposed assumption-of-risk and ordinary-negligence release | Pennsylvania attorney and insurer review; no enforceability claim |
| Privacy notice and optional data sharing (PDF) | Actual record handling and independent optional sharing choices | Complete secure return/storage, access, retention, privacy contact and request procedures; do not assume HIPAA applies |
Official screening, kept separate
| Publisher document | When it is relevant |
|---|---|
| Official 2025 PAR-Q+ fillable PDF | Follow the complete official questionnaire and applicable follow-up pathway; the original health-history form above does not replace it |
| Official 2026 ePARmed-X+ physician follow-up PDF | For the appropriate clinician when indicated, not an automatic requirement for every client |
Version check: October 26, 2026. The publisher identified the 2025 English fillable PAR-Q+ as current. Check the current-version page before use. Follow the full pathway rather than treating any “yes” as automatic exclusion or a completed form as blanket clearance for maximal exercise. Do not change prescribed medication for an assessment without the prescribing clinician’s advice.
The publisher’s conditions of use distinguish administration, reproduction, commercial use and digital integration. Unmodified-photocopy permission is not a blanket license for commercial rehosting or an electronic questionnaire. Resolve intended-use permission; we link to official files rather than copying their questions.
Coach records
These are the primary record templates. “Coach” describes who prepares them, not access control on the blank downloads. Completed records must be held privately, not on this website or in a public repository.
| Original review draft | Use it for |
|---|---|
| Lactate assessment checklist and session record (PDF) | One complete assessment: screening/consent references, equipment and QC, agreed protocol, stage readings, cessation/recovery, interpretation and reporting |
| Site-specific emergency action plan (PDF) | Venue, EMS access, communications, actual AED access, named trained responders and rehearsal; resolve solo-coach role conflicts before testing |
| Incident and near-miss report (PDF) | The single detailed incident record: observed/reported facts, actions, notifications and follow-up. Immediate care and medical evaluation take priority |
| Blood exposure-control plan (PDF) | Applicable exposure determination, safer devices, PPE, cleaning, sharps/waste, training and prompt clinical post-exposure arrangements |
These records do not replace an operating procedure, practical training, legal authorization or a rehearsed emergency response. Finger sampling is the scope of this packet; it does not authorize earlobe sampling or override current device instructions. No prescribed workload progression or automatic threshold algorithm is supplied.
Optional working formats, not extra required forms
Choose these only for a distinct need; do not ask someone to complete the same information twice.
| Format | When to choose it |
|---|---|
| Minimal emergency-contact worksheet (TXT) | Only when a separate contact record is requested instead of using the contact section already in the health-history form; not screening or treatment consent |
| Quick pre-test readiness worksheet (TXT) | A short preparation aid when the full session PDF is not being used for that check. It is no longer a second incident form; use the incident PDF above |
| Lactate-stage log (CSV) | Structured stage data for spreadsheet analysis or an identified attachment to the session record; not a replacement for its screening, QC, recovery and interpretation sections |
The CSV has no calculations, automatic thresholds or TrainingPeaks import. Use one identified source of stage readings and reference it from the session record rather than retyping competing copies.
Using the fillable PDFs
Download a local copy in an AcroForm-compatible reader. Save, close and reopen it to check that entries persist; viewer support varies. Long answers need an identified private continuation sheet. Typed names are not authenticated electronic signatures. The PDFs do not submit data, connect accounts or provide a signing audit trail. They are not claimed to be PDF/UA-certified.
Professional reference shelf
These publisher examples informed review; they are not additional client forms to complete or substitutes for the original drafts above.
- ACE official forms library: Health-history and other professional examples; publisher access and reuse conditions apply.
- ACSM sample chapter (PDF): Box 2.1 on printed page 11 is a clinical consent example, not a ready-to-sign coaching form. Do not copy its diagnosis, ECG, emergency-care or medical-record promises into a nonmedical service.
- OSHA emergency planning reference: supporting guidance for a real site-specific plan.
- OSHA model plans, Publication 3186 (PDF) and official index: the 2003 model needs review against the current bloodborne-pathogens standard. Covered employers with occupationally exposed employees have specific duties; this library does not determine a solo operator’s status or other applicable obligations. OSHA Form 301 is not a general client-incident form.
No third-party reference PDF is rehosted here. A signed packet is not operational clearance: scope, suitability, sampling authority, insurance, facility arrangements and secure records handling still need to be resolved before testing.
